Mail merge

Could someone please create an example so we can grasp the idea of mail merge in Zoho Creator.

I have a for with Name, Address, Telephone No and Email.

I have a form with a multi line field that I want to include a mail merge so each email is personalised with the name and address.

I then want the text in the multi line to be emailed to the correct email for each recipient.
