Mail Merge Doc problem

Hi,

I have a document loaded in to the mail merge templates. The problem is it is only one page but when we merge and make PDF it is over 2 pages ( The second page is blank).

The other problem is, when you go to edit it in the online editor, all the formating and layout is lost.

I've attached the document.
