Mail Merge Envelope Formatting

I can set up a mail merge template that is just a #10 envelope and get it working properly and save it. However, once I close it and reopen it the format is changed. In particular, the header and footer are back which need to be removed to make an envelope print properly. And the indents for the address fields are all messed up. I have similar problems with other 8.5 x 11 mail merge document although there it shows up as additional lines being added into the header and footer when save then close then reopen document.
