Improve Mailchimp integration by adding functionality to sync multiple fields (beyond email and name/or one other).

Currently this makes the extension unusable, to run even a basic email campaign you need to map and sync more fields than this.

It doesn't even need to be custom fields, just the fields that Mailchimp offers as standard.
