Module Association after Lead converted to Contact

Module Association after Lead converted to Contact

I've set our Form entries to be entered as records in a custom module.  What I'd like is for the record to associate automatically with the lead or contact that matches the email address on the form.  Then if that lead is converted to a contact the record association would continue on.
