Moving Form Data to Cart for Check Out

Moving Form Data to Cart for Check Out

My product needs configuration such that 3 levels of attributes aren't enough (plus I need file upload capability).  The form capability would work just fine.  Is there a way to move the Form Data to the Cart for a check out?  It does not look like Zoho Flow supports anything like that.
