My form won't create a lead or send the automated email?

My form won't create a lead or send the automated email?

I've created two forms according to the instructions. But when I fill in the fields to test it and hit Save, it goes to the next page but I can't find the new Lead details that should have been created and I don't receive and email? I even just made a form with first name, last name and email, and it won't work from the get go? What gives?
