Does anyone know how to populate with Zoho CRM and a Word Document populate?

How do I populate a word document from Microsoft word and integrade a specific Customer's infomation from the ZOHO CRM?

Please see the document I attached which are the fields I need to populate?

In addition, do we need Microsoft Office Professional Plus or just Microsoft Office Professional - - and which version (2010 or 2013) we have 2010 on our computers and its really important that we are able to get all of our customers information into this document from our CRM - please help!!!
