Potentials view all scrambled

Please see attached screen grab from nowhere my potentials are all scrambled. I want to change the stage of the potential but if I select the drop down box stage in the image is doesn't show my preconfigured stages and gives me an error. I have used this product for 2 years with no problems now this???
