Pre-populating a CRM Integrated Survey with a Respondents Information

Hi, 
I'm having trouble with the following workflow:

Open Contact in CRM>Send email w/survey Link

Respondent clicks Survey link>Survey open with their information pre-populated

Here are the fields I have integrated:


I'm unsure of what to tell Survey the Parameter Name is:


Here is the survey link the CRM generated for me when creating the email Template:

How does this come together? What should I be using to pre-populate the fields?
