Price field not accepting multiple characters

Library Materials Request Form:
Can you change the Price field to accept multiple numbers with decimals. 
Currently it doesn't accept more than 2 numeric characters.  For example, if we enter 110.00 it will only accept 11. Nothing more.
I would make the change myself but every time I change a field, the form disappears (as you may know from previous treads)

Another question:  Would creating a whole new form alleviate any problems with the form disappearing when I make an edit.  I've had it disappear 4 times and always have to contact customer support.
