Problem using workflow rules with potentials

Hi

I am trying to create a workflow rule that assigns a task when a potential stage is changed to '1st appointment'. This is what i have created for the workflow rule:

1.Created a workflow rule for 'potential' record types
2.Execute this rule when new records are created and existing records are edited.
3.Stage contains 1st Appointment
4.Rule is active
5.Created a task under this workflow rule for 'potential' record types
6.Created a task under this workflow rule with due date of 'WFRule Trigger Date Plus 0 Days'
7.Task shows under 'related tasks' table for the workflow rule

I have tried creating a test potential and then changing the stage to '1st appointment' but the task is never created. Is there something I am missing?
