Problem with Captcha on Web to Case Form

I cannot seem to get the Web to Case Form captcha to work properly. It just keeps showing the captcha form field, but not the code. I put the three scripts in my page per the instructions, but I think the script code is in the wrong place. I tried it in a couple of spots, but keep getting the same result.

Any thoughts? Can anyone tell me exactly where to put the <script> code in my Web to Case Form? The instructions just say to put it before the </div>. That could be anywhere in the form after the <div> at the beginning.
