Problems with Custom Fields and Field Layout - Accounts

Hi,

I am working on a customer implementation and having issues with custom fields and layout showing.
I have created some custom fields in the account section (see below)



I have then created a custom page layout for the account page(below):




I have saved the layout - however, the custom layout does not show when you create a new account(below):



Please help?
