Tasks and Events not registering

Tasks and Events not registering

When I create a task or even for either a lead or a contact, it does not register with that lead or contact. I can see the task/event under the activity screen, but when I click on it, its not assigned to any lead/contact. Also inside the lead and/or contact, the task/event just created does not show in "Open Activities"
