This might be silly or it might be genius

I have created a PDF editable (so my clients can fill in the blank information) using abode acrobat Pro. I would like to know if there is anyway to embed Zoho CRM into that form so that no matter who fills it out it or where it is downloaded from then hits submit in the PDF it will be sent to my CRM. Basically helping me eliminate data entry even further.
