Steps to add questions to your booking form:
Select Services on your Bookings menu bar.
Choose the required service from the left sidebar. Then select Service Booking Form.
Click the Add Field button in the right corner.
In the dialog box, enter the Field Label and choose the Field Type. Select the Mandatory checkbox if you'd want to ensure customers fill out the field.
Click Save. You can preview your booking page to see how it looks.
