It is not possible to add tooltip or multiselect lookup fields in CRM layout, the option is simply not there when editing the field properties.

Could this be added?

Similarly some system fields such as Name (i. e. Campaign.Name, etc.)  do not have the edit properties option at all which is understandable though having the ability to add a tooltip here should not cause any trouble.
