Use custom CRM fields to create segments in lists

How do I use the custom crm fields i made to create segments in the campaign lists/syncs?

Also can I create lists based on crm field criteria and have the users unsubscribe from each new list at a time instead of an opting out of ALL emails with one click?  We offer a large variety of email content ie. sales, product info, events etc
