Web to contact form no longer working after joomla upgrade.

We recently upgraded our website to Joomla 2.5 and the developer copied over the code for our web to contact. I've tried re doing the web to contact form but its still not working. I fill in the contact form on the website but the info doesn't show up in the leads section. 

Any suggestions / help would be greatly appreciated.
