Web to Lead - Broken again?

Hello,

I've just noticed an issue with the Web to Lead feature. This might be a configuration issue on my side. Not sure. I have not changed anything for months.

The email confirmations are working correctly but the lead data is not getting stored to the CRM DB. Ugg!
