Web to Lead email notification not using correct template

I have a newly created web form on my site with a custom email notification template selected.  However, when the form is processed it is using the custom email template from a separate web form on the site (but resides on a different page.)  How to ensure my web form is sending the proper email notification as selected in the setup?
