Web-To-Lead Form - Cannot get it to work?

Above is the link where I have embedded the Web-To-Lead Form. I need help with two issues here:

1) I have specified the return URL as http://www.jobsxs.com, but it keeps going to http://www.manageengine.com/.

2) Entries made in the Web-To-Lead Form does not update Zoho CRM.

I am not much of a techie, so it is possible that I may have done something incorrectly here. Please let me know if I have done something incorrect.

I need to sort this out quickly because we are in the process of rolling out our "Internet Marketing" strategy, and Web-To-Lead Form plays a major role in it!

I have also attached a copy of the HTML page that contains the Web-To-Lead Form, just in case you need to have a look at it.
