Web to lead form doesn't generate lead in CRM

I've installed the code for a web-to-lead form on my website. When I test the form, it redirects to the thank you page I specified, but no lead is generated in my CRM. I've searched for similar topics, and have already checked the following:

1) I do not have it set up to require leads to be approved. The web form does not generate a lead in the approval queue.

2) The domain names in the form code are set to https:// instead of http://
