Web to lead form has stopped working

Web to lead form has stopped working

We have a web to lead form on our website for people interested in becoming customers here that was working perfectly but now has stopped working suddenly. People can fill in their details, but they don't then appear as a lead in the CRM.

I'm very concerned about this as we are probably using business. 

Please help!
