Web to lead form not submitting

Hello,

I have added the web 2 lead form as a test to our server: http://localseoexperts.ca/new-lead.php

When form data is submitted, it do not show up in my leads area,

I have not made any edits to the code (page above contains 100% provided by crm )

I appreciate any assistance.
