Web to Lead Form Not Working

We have a form setup on our site at www.burginconstructioninc.com/contact that is not working. We have redone the form and readded the code, but that has not fixed the issue. Customers are able to complete the form and click submit, but this does nothing and they are not added to Zoho CRM, nor are we notified that they completed a form. Please advise on a fix for this issue asap as we are losing prospects while this portion of our site is down.
