Web to Lead form not working anymore.

I'm having issues with my web to lead form from my website http://www.evolvegymoc.com. My website guy had created a slightly different look to the Zoho form, but had all the proper coding regardless. This was working forever and was generating leads into my Zoho until 3 months ago. He has changed nothing on the site and I have not touched my Zoho account settings. Can anyone help me?
