Web to Lead Form Troubleshoot

I am using the free version of Zoho to check out how the functionality works, and there appears to be a slight hang up on the web-to-lead form functionality.

the page I'm working on is http://beta.eduglobalstudies.com/?page_id=40. When I submit the form, the form does not enter the data into the Zoho account, and it does not redirect to the Thank You page.

Any idea why this could be occurring? I did make some custom tweaks to the form, but nothing severe that should have messed up the form. Any ideas?
