Web to lead - not working

Hello,I created the form as described in the help section.
I integrated it into my website and it appears the way it should.
Nevertheless. when I fill in the fields, and press submit, I cannot see the information into the CRM database not receive the email as setup.

Any ideas or suggestions ?
