Web to Leads Form - Does not save in admin area.

I have noticed an issue when trying to create a new "Web to Leads Form" or edit an existing one.  When I make any changes or create a new one and hit the "Save & Next" button it just goes to a white page and does nothing.  It has been this way for about a month now.

I'm currently using the latest version of Chrome (I have used Firefox and IE9 as well) and all other functionality in the admin area seems to work as normal.
