Web to Vendor Form

I would love to be able to make a Web to Vendor form.

I get a lot of inquiries from people that would like to work with me, or to provide their services to me, and I'd love to be able to direct them to a Web to Vendor form, so that I can get them in the right place.

Currently I have to manually enter all the Vendors and potential Vendors.

Any way to do this ??
