I am setting up a web form from Zoho CRM Cases module to embed on our Zoho Site to allow customers to request support.

The problem I am facing is I cannot see a way for the customer to tell us the business name (Account) or leave a phone number to allow us to contact them.

There are no fields in the left pane - "fields" or "advanced fields". 

Is there a way to create a field for the contact's phone number (#) and Business Name (free text)?
