We have 3 different users who are experiencing the same problem.  When they update information on an account, a new account is being created with a different account name containing special characters.

Example:
Account "John Jones & Company" Exists... After updating information we now have existing Account "John Jones & Company" but also "John Jones&Company".  It's not limited to "&"...  Also have one that was "Tom Smith" and created another account "Tom Smith"...
