In order to check the delivery status from our end, we request you to send an email with the signup form URL (You can get this by opening the signup form in Zoho Campaigns) and the email address used for submitting the signup form along with approximate time of submission to support[at]zohocampaigns[dot]com

I am having this same issue. Is there an answer for this? I have associated the form with a list and I have enabled sign up forms. I have tried using the default URL and the embedded code. Nothing seems to work.
im having a similar issue, i can submit forms fine on desktop - when i test mobile on android or iphone it just goes to a blank page: http://zc.vg/7Nonq
