Wordpress Contact Form to Zoho CRM

I was looking for a WordPress contact form so that my clients could easily insert a contact form on their sites and have the new lead automatically entered in their Zoho CRM.  When I couldn't find one, I decided to write it myself using the API.
I built it on the Ninja Forms plugin, which is free and available directly from WordPress, because it has built in data validation and other extensions that really take advantage of Zoho's power.  For instance, you can automatically insert User Analytics such as browser, geographical location, and IP address into your CRM account.
