Zoho CRM - Custom Module - Post data from external application

I would like to know if it is possible to post date from external application to a custom module in some way. The custom module fields and post parameters are same.

I am assuming a web form to capture the value of the custom module. And then post the data to data from external application to custom form. Will it work? Any idea?
