Creating the Form
First, you can construct a form (as shown below) to collect basic wellness information such as age, height, weight, calories burned, and calories consumed.
Configuring the Field Rules
You have to show “Need a Diet Plan?” field only when the calories consumed is greater than the calories burned. Here's how you can configure this using Field-to-Field Comparison in Field Rules:
Navigate to the Rules tab.
Click Configure Now under Field Rules.
Click Switch to Advanced Rules to use Field-to-Field Comparison.
Select the Primary field as Calories Consumed (Daily).
On the right, click the icon to select the secondary field — Calories Burned (Daily).
Choose the Dependent field as “Need a Diet Plan?”. This field will appear on the live form only when the condition is satisfied (i.e., when calories consumed > calories burned).
Click Save for the rule to take effect in the live form.
Here’s how the form appears for Richard, whose calories consumed exceed the calories burned:
This form can be further customized and used across a wide range of wellness programs and demographic groups. For instance:
Corporate wellness initiatives targeting employees with sedentary work routines
University or school health check-ups to promote healthy habits among students
Fitness centers or dietitians collecting baseline data to suggest personalized plans
Public health campaigns run by NGOs or government bodies for awareness and intervention
Preventive care assessments in clinics focused on lifestyle-related risks
You can further enhance the form by including conditions for identifying those who consume significantly fewer calories than they burn—potentially leading to underweight issues. Additionally, the form can be extended to collect details like medical history, allergies, and current medications, ensuring that dietary recommendations are safe and tailored to the individual's needs.
