My website and form are in the same domain, but my sessions and source/medium values are incorrect in Google Analytics.

When website and form are in the same domain, the form will automatically inherit the website's parameters. Make sure you do not have any UTM tracking code inserted into the webpage. This would alter the values of the source/medium.
