How to create a card form?

1. If you are creating your first form in Zoho Forms, click Get Started.
   
2. Click Blank Form to create your form from scratch.
   
   
   
3. Under Create From Scratch, select the Form Type to be Card.
   
   
   
4. Enter an appropriate name for your form in Form Title  and click Create. You will land on the form builder for a Card form which will look a lot similar to the Standard Form builder. Know more about the Standard Form builder.
   Each field that you drag and drop into the Card form builder will be displayed separately in different sections
   
   
   
   

Welcome Page

1. Click + Welcome Page on the top. You will be taken to the Welcome Page Settings.
   
   
   
   
2. Select a layout that decides the placement of the image/ video and content on the Welcome Page. 
   
   
   
   
3. Click Upload Image/Embed Video. A pop-up where you can see the option to upload an image or embed a video onto the Welcome Page will open.
   
   
   
   
1. If you choose to upload an image, you can drag and drop/upload your brand logo or an image that suits the idea of your form well. You can either choose to keep the image above or below the content.
   
   
   
   
   Using the options in the image's top-right corner, you can change its alignment, or delete it. jhg

 Embedding Video in a Card Form

To copy embed code from Zoho WorkDrive

1. Select the file in Zoho WorkDrive.
   
2. Click the Share dropdown in the top action bar.
   
3. Select New embed code from the dropdown. 
   
4. From this copy the source alone as highlighted in the below example.

5. Paste this code in the Embed Pop-up in Zoho Forms.
   

Note: For WorkDrive URLs with custom domains, you'll need to modify the URL format by replacing the custom domain with workdrive.zohoexternal.(dc).

Supported video sources:

YouTube, Instagram, Vimeo, Loom, Dailymotion, Wordpress TV, KinoMap, Zoho WorkDrive, Dubb (copy the source URL from the embed code for Dubb).

Supported audio sources: 

Spotify, SoundCloud, Amazon Music, Youtube Music, Apple Music, Zoho WorkDrive.

         4. Add content to be displayed on the Welcome Page. You can apply basic text formatting to this content.



     5. On the Welcome Page, there will be a Start button that, when clicked, will take users to the first question. You can rename this button as required by clicking the edit icon.
     6. Click Save in the top-right corner.

The theme that you choose for your Card form will be applied to the Welcome Page while you edit it.

Note: The option to configure Welcome Page in Card forms is available only in our paid plans.

Form Properties

1. Click the Form Title to view the Form Properties pane.
   
2. Here, you can change the Form Title, provide a description and give a nickname to the form for your reference.
   
3. Under Display, you can choose to show/hide:
   
   
   
   
1. Form Title either on all the pages or only on the first page with the first question.
   
2. Quick Navigation that lets respondents jump between questions instead of having to navigate to the previous or next page to get to that page.
1. Progress Bar that lets respondents know how far they've progressed through the form.
2. Field Sequence Number that gives the sequence number of each form field.
   
4. You can also change the labels for page navigation buttons ( Next and Prev ).
