For this, when you log on to Zoho Survey - Offline Survey app for the first time, or when you do a first update of the product (for existing users), you'll be prompted with the following screen asking for your consent. You can also change the permissions you have entered at a later point by accessing the Data Permissions option in the Dashboard.
