Email Marketing - Competitions Entries

We are going to be holding a competition, and wouild like to use the campaigns to create a form with tick boxes etc to select their answers to the questions we have.

Is it possible that the specific inventory questions can be automatically mapped to the relevant fields in CRM?

How can we create this?
