Email Recipient Activity stated as "Unopened"

Email Recipient Activity stated as "Unopened"

Hi,

I received feedback from recipients that they did not receive the email (both Inbox & Junk mail).

However, it shows on the email recipient activity report as unopened. 

Has the email been delivered? Or was it lost somewhere in the email server?

If it has been delivered, why doesn't it show up in the Inbox or worst case scenario, their junk mail?
