embed multiple paged form into website

I've embed a form onto a web page to collect data. We've created three forms that are linked via scripted and pass the "email address" field through to ensure continuity of the data.

This issue is the second and third pages (forms) have my logo, sign in, feedback, form name, etc. on the page. I'd like just the form, like the first page. How can I call the next pages without having all the extra stuff.
