Form is blank after import

I have made a copy of an existing app and added some bells and whistles to the main input form. I have then exported the data from the original app and imported it into the new version using the migration tool. All of the information is there in the database report, but if I try to edit, view or print any record, the form just shows up totally blank?

Please help!!
