I not able to submit sign up form.

hi,
I having few problems in setting up my account

1.  I not able to submit sign up form. Nothing happens when clicking.


2. Not able to integrate CRM with campaigns.please see the attachment.

3. After embedding sign up forms in landing pages, CRM not tracking and recording data.
