Hello,

I would like to have a public page, where users can enter product registration details.  This needs to be many-to-one (registrations-to-contact).  I tried the CaseToWeb Form.  It adds a Contact and Case, which is fantastic.  However, it will only let me add a few details on the Contact such as Name, Phone, Email.  I also need the ability to add address, city, state, zip fields.

Has anyone tried this before?
