Subscribe Form does not work

I am trying to create a sign-up form for an e-mail list in Zoho Campaigns. I want this form to be accessed from a web page I have hosted in Zoho Sites.

I used the wizard to create a button as well as a form, but in each case, the actual button to submit your data doesn't do anything. Is anyone else having trouble?
