I work for an organization that is installing 88 pianos throughout 88 locations around my city. Each piano has a buddy. These buddies need to be able to access a form to let us know of any problems with the pianos (ie. missing keys etc). This form needs to then be submitted to a master list, so we can identify problems for our maintenance team in a sufficient manner. The form needs to be similar in style to the website that already exists for our organization. Basically when you click on a tab on our website we want this form to appear. It needs to be as user friendly as possible.
