Checkbox Field - Candidate Application Form

Hi,

How can we add the above as another field in the candidate application form? Candidate might be required to make declaration using this field regarding the information they provide, that is the info are true and understand the consequence should it be found out that documents and info provided are falsified or forged. Thus the need to add this field in the application form.
